چکیده

Introduction Substrate-based radiofrequency (RF) ablation of scar-related ventricular tachycardia (VT) has become standard practice, often in addition to an intracardiac cardioverter-defibrillator. VT procedures without identified substrate are particularly challenging cases, especially when VT is noninducible. An intramural focus is often challenging to ablate using conventional RF ablation, and different options are or will be possible such as high-power unipolar ablation, bipolar ablation, irrigated needle ablation, or selective coronary ethanol injection. The following case is used to illustrate a patient-tailored approach to choose between bipolar and ethanol ablation after a failed RF attempt, since needle ablation is not approved yet for human use.

What Next After Failed Septal Ventricular Tachycardia Ablation?

Ablation of ventricular tachycardia (VT) by conventional radiofrequency ablation can be impossible if the ventricular wall at the targeted ablation site is very thick, as for example the ventricular septum. We present a case of a patient with incessant, non-sustained slow VT originating from the septal part of the lower outflow tracts. Radiofrequency catheter ablation from both ventricles as we...

High-power bipolar ablation for incessant ventricular tachycardia utilizing a deep midmyocardial septal circuit

A 68-year-old man with cardiomyopathy out of proportion to ischemic heart disease, previous coronary artery bypass grafting, and a biventricular implantable cardioverterdefibrillator presented with incessant ventricular tachycardia (VT) refractory to amiodarone and previous ablation attempts. Recent nuclear stress testing demonstrated no evidence of ischemia and a small area of scar at the infe...

Successful bipolar ablation for ventricular tachycardia with potential substrate identification by pre-procedural cardiac magnetic resonance imaging

Cardiac magnetic resonance imaging (MRI) is a useful tool for detecting the arrhythmogenic substrate in cardiac sarcoidosis. We herein present a case of bipolar radiofrequency catheter ablation for ventricular tachycardia (VT) complicated with cardiac sarcoidosis, guided by pre-procedural cardiac MRI. Neither echocardiography nor endocardial voltage mapping suggested a septal VT substrate. Howe...

Characterization of trans-septal activation during septal pacing: criteria for identification of intramural ventricular tachycardia substrate in nonischemic cardiomyopathy.

BACKGROUND Identification of intramural basal-septal ventricular tachycardia (VT) substrate is challenging in nonischemic cardiomyopathy. We sought to (1) characterize normal/abnormal trans-septal right ventricular (RV) to left ventricular activation; (2) assess the effect of opposite RV pacing on left ventricular septal bipolar electrograms (EGMs); and (3) establish criteria for the identifica...

Upper septal transformation of verapamil-sensitive idiopathic left ventricular tachycardia during catheter ablation: Wolf in sheep׳s clothing?☆

Introduction Verapamil-sensitive fascicular ventricular tachycardia (VT) is the commonest form of idiopathic left ventricular tachycardia (ILVT). The characteristic diagnostic features of fascicular ILVT include: (i) induction with atrial pacing, (ii) right bundle branch block (RBBB) with left-axis deviation morphology during VT, (iii) absence of structural heart disease, and (iv) verapamil sen...
